Name field

The Name Field in Formester allows you to collect a respondent’s name in a structured way. Unlike a simple text box, this field lets you break the name into First, Middle, and Last Name sections, giving you more flexibility and cleaner data.


Why use the Name Field

Unlike a single line input, the Name Field keeps responses organized and consistent. It’s especially helpful when:

  • You need official details like first, middle, and last name
  • You want clean data for personalization in emails or reports
  • You’re building formal forms such as job applications, registrations, or onboarding forms


Open your form in the editor

Go to your Formester dashboard and select the form you want to edit.


Add the Name Field

Click Add Elements and select Name under Personal Fields. By default, the field shows First Name and Last Name.


Customize field options

Toggle on or off the First, Middle, and Last Name parts. This gives you control over how much detail you want.

  • For a casual survey, keep only First Name
  • For a registration or official form, include all three

Set a default value if you want the field pre-filled. For example, you might use “Guest” as a placeholder when testing or to reduce friction for repeat users.


Configure validation

Mark the field as Required if the name is a must-have detail for your form.

Add placeholders like “Enter your first name” to guide users and make the field more intuitive.


Best use cases

  • Event sign-ups where only First and Last Name matter
  • Job applications that require full legal names including middle names
  • Customer intake or onboarding forms where personalization is important
  • Feedback or surveys where First Name alone is enough


Publish and preview

Click Publish once you’re done setting up. Always use the preview option to make sure the Name Field looks correct and behaves as expected.
